We've decided to improve this situation by allowing students who get "kicked out" of a quiz/test to re-enter and properly complete it. This feature is a Beta release and must be enabled on the Setup tab to be available. See details below.
To resolve this issue, we've implemented an inactivity timeout. If no session activity is detected within an hour, the session is automatically closed. If the session includes an open quiz/test, then the quiz/test is handed in and graded as is.
NOTE: The session timeout includes instructors as well as students. This means that if you are signed in to the Management area and don't do anything for an hour, you will be signed out.
We've updated the Add Module feature on the Course tab to prevent users from adding the same module twice to a single course. This update does not modify current courses; it only prevents the issue from occurring on future occasions.

When the student signs back in, (s)he is automatically redirected back into the same quiz/test where it can be finished.
If the student does not sign back in within an hour, the quiz/test is considered inactive and it is automatically handed in, and graded "as is". In this case, the quiz/test can be reset, per existing procedures.
In short... If a student is "kicked out" of a quiz/test due to user or network errors, simply have the student sign back into the system. The student is redirected back into the same quiz/test where it can be finished and properly handed in.
As noted, this is a Beta feature that has to be enabled. Use the following procedure to enable this feature:
To disable the Beta feature, simple clear the Enable Quiz/Test Continuation option and click Update.
